  • Her Power Rx Health Solutions operates as a DBA of Revived Beauty LLC, offering durable medical equipment and services, primarily for female veterans and women, specializing in custom-fit cranial prosthesis prescription wigs.

  • These terms apply to all customers, patients, and users of our services, including products, websites, and portals.

2. Privacy and Data Protection

Your privacy is our priority. We are committed to using HIPAA-compliant systems to protect your personal and medical information. Please review our Privacy Policy for details on how we handle your data. By using our services, you consent to the collection, use, and storage of your information as outlined in the Privacy Policy.

3. Use of Services

  • Our services are intended for individuals seeking durable medical equipment and prescription-based products.

  • Prohibited Uses: You agree not to use our services for any unlawful purpose, to misrepresent yourself, or to engage in any activity that could harm the company or other users.

  • All communications with Revived Beauty LLC and Her Power Rx Health Solutions must be conducted through approved channels, such as our patient portal, email, or phone. Unauthorized communication methods may result in termination of services.

4. Medical Disclaimer

  • Our products, including cranial prosthesis wigs, are classified as Durable Medical Equipment (DME) and are designed to meet medical needs, not cosmetic preferences. We are not personal stylists, cosmetologists, or hair professionals.

  • All services are provided based on valid prescriptions and patient-provided measurements.

  • A custom-fit product is not a guarantee of perfection, but we strive to meet your specifications as closely as possible.

5. Custom Orders and Prescription Products

  • Custom Orders: All prescription products are made to order and are non-returnable and non-refundable.

  • Measurements: It is the responsibility of the patient to provide accurate measurements. Any errors in measurements provided by the patient may result in additional charges for adjustments or corrections.

  • Processing Time: Orders are processed within 2 business day of form submission. Delivery times range from 4 to 5 weeks unless otherwise specified.

  • Cancellations: Once production has begun, cancellations are not permitted.

6. Returns and Exchanges

  • Custom prescription wigs and cranial prosthesis items are non-returnable.

  • Non-custom items may be returned within 3 days of receipt, subject to a 20% restocking fee. Items must be in original condition, unworn, unaltered, and with all original tags and packaging.

  • For eligible exchanges, customers must contact us within 3 days of receiving the item. Shipping costs for returns and exchanges are the responsibility of the customer.

7. Payment and Billing

  • All payments must be made in full before production begins. Accepted payment methods include credit cards, debit cards, and approved insurance payments.

  • Customers are responsible for ensuring their payment information is accurate and up to date. Any issues with payment may result in delays or cancellations of services.

8. Limitation of Liability

  • Revived Beauty LLC and Her Power Rx Health Solutions are not liable for any loss, damage, or theft of products after delivery is confirmed. Proof of delivery (e.g., tracking confirmation, photographs) is considered final.

  • We are not responsible for misuse or mishandling of products by the patient or the customer.

9. Intellectual Property

  • All content, branding, logos, and materials provided by Revived Beauty LLC and Her Power Rx Health Solutions are protected by copyright and trademark laws. Unauthorized use or reproduction is prohibited.

  • Customers may not copy, reproduce, or distribute any materials without prior written consent.

10. Compliance with 10DLC and Communication Policies

To maintain compliance with federal communication standards, all text and email communications are regulated under the 10DLC framework:
